This PDF form must be downloaded and completed using Adobe Acrobat Reader (make sure you have it installed on your computer). Follow the instructions below to save the PDF form to your computer then open it. Fillable forms cannot be completed on mobile devices.

1. Right-click on the form link and select:
   • - Save Link As (if using Chrome, Firefox or Edge); or
   • - Download Linked File As (if using Safari)
2. In the new window, select Desktop then click Save,
3. Locate the form on your Desktop, right-click and select Open With, then select Adobe Acrobat,
4. You can now complete and save your PDF form.
